For over 30 years, Anne Grogan has helped seniors and their families navigate health and social issues. Anne earned her Master of Social Work (MSW) from Marywood University, and is also a nationally recognized certified case manager (CCM) through the Commission for Case Management Certification. In addition to being a Licensed Social Worker in PA and NJ, Anne holds certifications in elder law, bioethics and public health, and a faculty fellowship in geriatric education.
Anne was the Director of Geriatrics for a large hospital network for 30 years, designing, developing, and implementing programs to meet the needs of a growing elderly population. With her strong background in ethics she was asked to Co-Chair the hospital Bioethics Committee. An industry leader, she has been asked to speak at state and national conferences. Recognizing the importance of having well trained staff caring for our
older adults, she designed educational and training programs for senior living communities, home care agencies and the community caregivers.
Her entire career has been dedicated to creating and implementing programs to benefit seniors and counseling the “sandwich generation”
as they navigate the eldercare maze. In 2017, she opened her own private practice, working directly with older adults and their caregivers. Anne’s services are now available to families as a geriatric case manager to assess the needs of the older relative and develop a game plan to help the older person “age in place.”

WHAT IS GERIATRIC CASE MANAGEMENT?
A Geriatric Case Manager usually steps in when an older person and family are in crisis. GCM can also be a preventative service rendered on demand, increasing the quality of an older person’s life, coordinating all the players rendering services to the older person and offering assurance and peace of mind to the adult children of the older individual. GCM can be a life saver for the long distance caregiver.
